Cara Jakob

Dr. Jakob is a board-certified family physician. She completed her undergraduate studies at the University of Connecticut before obtaining her medical doctorate from the Drexel University School of Medicine.

Dr. Jakob is a board-certified family physician. She completed her undergraduate studies at the University of Connecticut before obtaining her medical doctorate from the Drexel University School of Medicine. She completed her residency training at Tallahassee Memorial Regional Medical Center. After completing her residency, Dr. Jakob moved to Clermont, Florida, and founded Total Family Healthcare. Her unique approach to medicine, which focuses on disease prevention and patient quality of life, is the cornerstone of her practice. Total Family Healthcare was among the first medical practices to adopt fully integrated electronic medical records. As Medical Director for Total Family Healthcare, she continues to see patients and offer treatment for substance abuse.

Dr. Jakob is also the co-founder of two accountable care organizations, Primary Partners and Primary Partners ACIP. Both organizations are wholly physician-owned and were among the first accountable care organizations in the state of Florida.

As the Clinical Integration Director for Primary Partners, Dr. Jakob works closely with local physicians, Orlando Healthcare, and South Lake Hospital to build partnerships with community stakeholders and facilitate communication between the hospital, nursing home, assisted living facility, social service agencies, and the community at large to advance better care for individuals, better health for populations, reduce duplicative procedures, and lower growth in expenditures.

In addition, Dr. Jakob is Chairman of the Patient Integration of Transition in Health Committee, which is responsible for researching, identifying best practices, and developing medical protocols regarding the delivery of patient care to acute care, long-term care, and ambulatory patient populations to obtain top quality care management and optimal physician, ALF, nursing home, and hospital alignment needed for clinically integrating medical protocols. The CID is responsible for sponsoring the High-Risk Patient Committee and the Patient Integration and Transition of Health Committee, which are dedicated to best practices and protocols for hospital, ALF, nursing home, and homebound care.

Dr. Jakob strongly believes in education and is a preceptor for Florida State, the University of South Florida, the University of Central Florida, and Nova Southeastern University. She is the recipient of several outstanding teacher awards and is known for being an innovator in her field as well as in her community.
