Dr. Deepam Gokal is a board-certified Internal Medicine physician with more than 40 years of experience caring for adults at every stage of life. Known for his thoughtful and compassionate approach, he is dedicated to helping patients feel heard, understood, and confident in their care. After many years serving the Reading, Pennsylvania community, Dr. Gokal now welcomes patients in Clermont, Florida.
He earned his medical degree from the University of the Witwatersrand in South Africa, beginning a long and rewarding career focused on providing dependable, high-quality care. Over the years, Dr. Gokal has developed extensive experience treating a wide range of health concerns—from everyday medical needs to more complex conditions involving the heart, circulation, and overall wellness.
Dr. Gokal has spent much of his career working in hospital settings, where he cared for patients during some of their most critical moments. This experience has shaped his ability to stay calm, thorough, and attentive, ensuring each patient receives careful evaluation and well-coordinated care from start to finish.
Before moving to Florida, he was part of Tower Health Medical Group in Pennsylvania, providing care at Reading Hospital and serving the local community for many years. Today, he brings that same level of dedication and experience to his patients in Clermont.

Dr. Christopher Duplain is a compassionate, board-certified family medicine physician dedicated to providing patient-centered care for individuals and families across all stages of life. A graduate of the Edward Via College of Osteopathic Medicine (VCOM) - Virginia Campus, Dr. Duplain completed his residency at HCA Florida St. Lucie Hospital. There, he developed a deep expertise in preventive medicine, chronic disease management, and continuity of care.

Dr. Cara Jakob is a trusted and experienced family medicine physician who has been caring for patients and families across Central Florida for more than three decades. At Total Family Healthcare & Wellness Centers, she is known for delivering thoughtful, relationship-driven care that prioritizes both medical excellence and genuine human connection.
Dr. Jakob earned her medical degree from Hahnemann University School of Medicine (now Drexel University College of Medicine)and completed her residency training at Tallahassee Memorial Hospital, where she developed a strong foundation in comprehensive, full-spectrum primary care. She is board-certified in Family Medicine and remains deeply committed to providing lifelong care to patients at every stage—from adolescents through adulthood and into the senior years.
At Total Family Healthcare, Dr. Jakob provides comprehensive primary care services designed to support long-term health and wellness. Dr. Bartley earned her Doctor of Nursing Practice from the University of Central Florida, where she developed advanced clinical expertise alongside a strong foundation in population health and healthcare systems. Her career began at Orlando Health South Lake Hospital in interventional cardiac care, where she gained valuable experience managing complex and acute patient conditions.
Building on that foundation, she has expanded her clinical scope across primary care, obstetrics and gynecology, aesthetics, and hospice medicine. Her training includes work with Orlando Health Primary Care, VITAS Healthcare, and the Lake Nona VA Medical Center, where she provided care to a diverse patient population, including veterans and those requiring end-of-life support.
